These Terms & Conditions ("Terms") govern your use of 5c.co and your participation in 5C's email and text messaging (SMS) programs. By using this website, submitting a form, or opting in to receive messages from 5C, you agree to these Terms. If you do not agree, please do not use this website.
1. About 5C
5C is a private, direct-pay medical practice located at 1831 N. Lakewood Drive, Ste 1, Coeur d'Alene, Idaho 83814. 5C is the operating name of 5 Codes PC.
2. No Medical Advice
The content on this website, in our email communications, on The 5 Codes Podcast, and on our social media channels is provided for general informational and educational purposes only.

Every patient is different. Descriptions of procedures, treatments, protocols, and outcomes on this website reflect the experience of specific patients and are not a prediction of your result. Individual results vary based on anatomy, health history, healing response, and other factors, and no particular result should be expected.
Any before-and-after photography shown on this website depicts actual 5C patients who have given written authorization for their images to be used. Photographs are not retouched to alter surgical or treatment outcomes. Photographs of one patient are not representative of what any other patient should anticipate.
4. Fees, Pricing, and Payment
- 5C is a direct-pay practice. We do not accept or bill health insurance, and we do not participate in Medicare or Medicaid.
- Any pricing, fee range, or estimate shown on this website or discussed before an evaluation is an estimate only and is subject to change. Your individual plan and cost are determined at consultation.
- Consultation fees, deposits, scheduling terms, and cancellation terms are provided to you in writing before you commit to care and are governed by the agreement you sign at that time, which controls over anything on this website.
- Because we do not bill insurance, amounts you pay to 5C are generally not reimbursable by an insurance plan. Any question of reimbursement or eligibility is between you and your plan or account administrator.
5. SMS Terms & Conditions
By opting in to receive SMS messages from 5C, you agree to receive customer care messages, account notification messages, two-factor authentication messages, and delivery notification messages.
Message frequency: Message frequency may vary. On average, 1-2 messages per month.
Message and data rates:Message and data rates may apply.
Opt-out: You can opt out at any time by replying STOP to any SMS message.
Help: Reply HELP for support. You may also contact us at (208) 215-7199 or info@5.codes.
Supported carriers: Carriers are not liable for delayed or undelivered messages.
Consent is not a condition of care: Opting in to receive text messages is never required in order to receive care at 5C, and opting out will not affect your care.

6. Marketing Text Messages (Separate Opt-In)
Marketing and promotional text messages are a separate program with a separate opt-in. You will not receive marketing text messages from 5C unless you have specifically consented to them, in addition to any consent you give under Section 5.
If you opt in to marketing text messages, you agree to receive promotional messages from 5C about services, events, and offers.

7. Email Communications
If you subscribe to 5C email communications, you may unsubscribe at any time using the link in any email or by contacting us. Unsubscribing from marketing email will not stop appointment-related or care-related communications.
8. Supplements and Third-Party Products
5C offers professional-grade supplements through a third-party dispensary platform. Orders placed through that platform are subject to that provider's own terms and privacy policy.
Statements regarding dietary supplements have not been evaluated by the Food and Drug Administration. These products are not intended to diagnose, treat, cure, or prevent any disease. Consult your health care provider before beginning any supplement, particularly if you are pregnant, nursing, taking medication, or managing a medical condition.

All content on this website, including text, photography, video, audio, graphics, logos, marks, and the 5C name, is owned by 5C or used with permission and is protected by copyright and trademark law.
